  • How to Apply for Medicaid

  1. Get an application. Choose one of these options:
    • Download and print the combined application. This application is used for Medicaid, DC Healthcare Alliance, food stamps, and cash assistance.
    • Apply at an Income Maintenance Association (IMA) service center. To find the one nearest you, call (202) 724-5506 or check this list.
    • Call 202-724-5506 to have an application sent to you.

  2. Fill out your application. If you need help:
    • Call (202) 724-5506. Free interpreters are available.
    • Ask a social worker or staff member at your IMA service center.

  3. Get your paperwork together. You will need to send this information with your application:
    • Proof of income
      (pay stubs or copy of your latest income tax return)
    • Proof of your assets
       (banking or checking account statements)
    • Proof that you live in DC
      (utility or telephone bill, lease, rent receipt, or copy of your DC driver’s license)
    • Proof of your Social Security number
       (Social Security card or other papers with your Social Security number on them)
    • Medicare card (if you have one)

  4. Turn in your application.
    • Mail the application to:

    • Or drop the application off at your IMA service center.

  5. Watch your mail. You should receive a letter within 45 days that will tell you whether you are eligible. If you are disabled, you will find out if you are eligible within 90 days.
