  • How to Apply for Healthcare Alliance

  1. Get an application. Choose one of these options:
    • Download and print the combined application. This application is used for Medicaid, DC Healthcare Alliance, food stamps, and cash assistance.
    • Apply at a DC Healthcare Alliance enrollment site. Find the enrollment site closest to you by calling (202) 724-5506 or check this list
    • Call 202-724-5506 to have an application sent to you.

  2. Fill out your application. If you need help:
    • Call (202) 724-5506. Free interpreters are available.
    • Ask a social worker or staff member at a DC Healthcare Alliance enrollment site.

  3. Get your paperwork together. You will need to send this information with your application:
    • Proof of income
      (for example, pay stubs or copy of your latest income tax return)
    • Proof that you live in DC
      (for example, utility or telephone bill, lease, rent receipt, or copy of your DC driver’s license)

  4. Turn in your application.
    • Mail the application to:

    • Or drop the application off at a DC Healthcare Alliance enrollment site.

  5. Watch your mail. You should receive a letter within 30 days that will tell you whether you are eligible.
